• TAMS Summer Orientation Registration

  • To be filled out by the Parent or Legal Guardian of an incoming TAMS student. At the end of the form, total pricing for the registered student and any guests will be tallied for the form filler's records. NO PAYMENT WILL BE TAKEN IN THIS FORM. Registration is due {calculation109}. The tallied amount will be applied to the incoming student's UNT account by {billingDate101}, and will be due by {paymentDue}.

    Please enter the requested information for the student that is entering TAMS in the coming school year. Base registration for students is $100. Registration is discounted to $25 for Early Summer Researchers.

    Registration is $80 for guests over 10 years of age.

  • IMPORTANT: Please read carefully before adding guests to this form:

    1. The table below should only contain information about guests who are accompanying {fullName} to Summer Orientation.
    2. TAMS Students may attend Summer Orientation without a parent/guardian present. Parent/guardian participation is highly encouraged, and there is a separate, unique schedule for adults.
    3. Please do not add additional incoming TAMS students to this table. 
      1. A new form must be filled out for additional incoming students attending Summer Orientation (e.g. a sibling who is also entering TAMS in the coming school year).
      2. If you are registering another student via this form, and that student is bringing the same guests as the previous student, please leave the table blank. 
      3. Registering the same guests in multiple forms will result in multiple fees being charged for the same guests.
    4. Children uner the age of 10 do not need to be registered.
    5. If the form filler will also accompany the student to Summer Orientation, the form filler must be added to the table below, unless exceptions are applicable (see Items 3B-3C above).
